Outpatient Youth Program
The Avanzar Wellness Outpatient Youth Program is a structured relapse prevention and skill-building program designed for youth transitioning from residential treatment or correctional settings back into their home and community environments. The program supports continued recovery, accountability, and pro-social development while reducing risk for relapse and recidivism.
The program operates on a continuous 16-week open-group cycle, with participants able to enter at any point in the cycle. Youth attend two 90-minute group sessions each week. Group sessions are curriculum-driven, addressing key areas including structure and routine, triggers and cravings, emotional awareness and regulation, stress management, peer influence, relationships and boundaries, values and motivation, life skills, recovery identity, and long-term relapse prevention planning.
Every fourth week includes a structured family session designed to strengthen family engagement, improve communication, rebuild trust, and align caregivers and youth around shared recovery goals. Family sessions focus on practical strategies for supporting recovery at home, reinforcing accountability, and maintaining consistency across environments.
In addition to group programming, each participant is supported by an assigned provider who delivers one-on-one services in the community. These services reinforce weekly group content and focus on skill generalization, helping youth apply what they learn in group sessions within their home, school, and community settings. Providers assist clients with learning and applying coping strategies, decision-making, emotional regulation, and relapse prevention skills in real-time situations, ensuring continuity between classroom learning and daily life.
The program emphasizes active participation, real-life application, and accountability. Youth are encouraged to practice skills through weekly challenges and reflection, promoting sustained behavior change, increased self-awareness, and successful community reintegration.
